Welcome Letter & Instructions
We would like to welcome you as a new employee to the Town of Brimfield. Please click here to reach out to the Treasurer for the most up to date new hire forms. It is necessary that these forms be filled out and returned as possible in order to be set up on the payroll system. Below is a checklist of the forms and supporting documents that we will need.
Documents that must be submitted:
- New Hire Information Sheet
- Federal Tax Withholdings
- MA Tax Withholdings
- Employee Eligibility Verification (make sure you include a copy of one of the documents under “LISTS OF ACCEPTABLE DOCUMENTS”)
- Social Security Form
- Nationwide OBRA enrollment form (fill out Sections I, III and participant’s Signature)
Optional:
- Direct Deposit Form need to be completed in person due to increased Security.
