Welcome Letter & Instructions

WELCOME, FULL-TIME NEW EMPLOYEE

We would like to welcome you as a new employee to the Town of Brimfield. If you would please take a minute and complete the forms that you will find in the “New full-time employee” section on the Treasurer’s page under Town employees. We need these forms filled out and returned to your department head as soon as possible in order to be set up on the payroll system.  Below is a checklist of the forms and supporting documents that we will need.


Documents that must be submitted:

  1. New Hire Information Sheet
  2. Federal Tax Withholdings
  3. MA Tax Withholdings
  4. Employee Eligibility Verification (make sure you include a copy of one of the documents under “LISTS OF ACCEPTABLE DOCUMENTS”)
  5. Social Security Form
  6. Hampden County Retirement Form (make sure that you fill out the beneficiary information and include a copy of your birth certificate)
  7. Either your health insurance enrollment form or a waiver form
  8. Either you life insurance enrollment form or a waiver form

Optional:

  1. Direct Deposit Form need to be processed in Person due to increased Security.
